You have expressed an interest in Puppy Kindergarten, Puppy II/Level 1 (under 6 months),
Puppy II/Level 1 (over 6 months), The Civilized Canine (CGC prep class).

Please fill in this form completely and return it to me along with your check for the amount of the class you are signing up for.

The first ten completed forms for each class will be given slots in the next series of classes that is scheduled to start on the date stated on the classes page, for the location you are signing up for.

Please bring a record of the puppy’s vacations from the vet to the first class.

You should also bring treats or a favorite toy for motivation.
